Becoming Medicare eligible and filling out Medicare forms go hand in hand. One of the first things you do is fill out Medicare Forms.
Conveniently, you can obtain many of the Medicare forms online by visiting the Medicare website.
Below we’ll discuss the different forms you may need, where to find them, and where to submit them.
An Explanation of Medicare Forms
The U.S Department of Health and Human Services has an agency within; this is the Centers for Medicare and Medicaid Services (CMS).
Forms are generally a requirement for enrollment, appeals, and change of authority. Filling out forms can seem daunting; but, most forms you can find online by downloading the PDF file.
Initial Medicare Forms
The first step when enrolling in Medicare is to complete an Initial Enrollment Questionnaire. The questionnaire informs Medicare about any other health coverage you may have.
Different types of insurance coverage could be employer benefits or veterans’ benefits.
Initial Enrollment Questionnaires are online through MyMedicare.gov. However, you can complete a questionnaire over the phone if you prefer.
You can expect a letter in the mail three months before your Medicare coverage begins. The notice will contain your MyMedicare.gov password, username, and instructions.
It’s important to remember your Medicare enrollment application goes to Social Security and not Medicare. Also, you won’t be automatically enrolled in Medicare.
You can apply by:
- Visiting your local Social Security office
- Social Security online
- Call Social Security. Representatives are available from 7 AM to 7 PM, Monday through Friday.
Those with Part A, need to apply for Part B of Medicare; you’ll need the Application for Enrollment in Part B form.

New to Medicare Forms
Once you pick the most suitable Part D plan, expect to fill out an enrollment form. Most enrollments can take place over the phone.
Medigap enrollment can also take place over the phone, but there is still a form; it’s just electronic and filled out by an agent.
Some people may opt for Medicare Advantage coverage; these enrollments also require forms. Just like other Medicare products, registration can take place over the phone.
Tip: Let an agent help you enroll, and you don’t need to figure out the enrollment forms alone!
Sign Up for Medicare Auto-Payment
Medicare Easy Pay allows you to have premium payments automatically deducted from your account. Although, if you’d like to enroll, filling out the Authorization Agreement for Pre-authorized Payments form is necessary.

How to File a Claim
Most of the time, you won’t file your claims. However, if you have a foreign travel emergency, you may need to submit a claim yourself.
There are other times you may need to submit your claim; however, it’s unlikely.
Although if you do need to file a claim, you can find the file online. The form is a Patient Request for Medical Payment.
Filing an Appeal with Medicare
You want to file an appeal when you don’t agree with a Medicare payment decision.

For those that don’t have a policy with us, you can find each appeal form online. Each level of appeal requires a separate form.
You may choose to appeal if:
- You can request to change the amount that you pay for a health care service
- Request payment for health services or medication that you received
- You believe your health care service or medication should be covered
You may decide that you want to appoint a representative on your behalf. In that case, you’ll need an Appointment of Representative document.
Should you need to transfer your appeal rights to a healthcare provider, you can fill out a Transfer of Appeal Rights form.
Medicare Forms You May Need During the Appeals Process
Appealing a claim is a process, and having all the tools you need is essential.
If you want to appoint a representative or transfer appeal rights, these are the forms you’ll need:
- Want to nominate a representative to assist you with the appeal? You need to fill out the Appointment of Representative form
- Want to transfer your appeal rights to a provider or supplier? You need to file a Transfer of Appeal Rights form
Depending on your appeal, you may need to file several times; each is different.
- The forms you could need include the initial appeal request form is the 1st level of the appeals process. The Redetermination Request form is a requirement.
- If you’re unhappy with the decision from the redetermination, you can submit the Medicare Reconsideration Request Form.
- Finally, if you’re unhappy with the 2nd level appeal, request a hearing by an Administrative Law Judge (ALJ). You can request this by filling out the Request for Administrative Law Judge Hearing or Review of Dismissal form.
Medicare Grievance Forms
The process for filing a grievance will depend on who and the issue.
Whether the issue is with a healthcare establishment or with your Medicare insurance company, if you wish to file a grievance about a doctor or hospital, you can contact the Quality Improvement Organization; also, you can contact the State Health Insurance Assistance Program.
Submit a Medicare complaint form to file a complaint about your Medicare plan.
Contact Medicare if you have an issue with a Durable Medical Equipment supplier or your reporting Medicare fraud. Although, if you have private Medicare Insurance, contacting your plan to discuss complaints is always an option.
An appeal and a complaint are two different things. Appeals are when you don’t agree with Medicare’s coverage decisions. Complaints are related to treatment and quality of service.
Medicare Authorization to Disclose Personal Health Information
Many times, a spouse will want their partner to have access to their personal health information. If this describes you, fill out the Authorization to Disclose Personal Health Information form.
This form is available online, or you can contact Medicare for the information.
Unavailable Medicare Forms

If you don’t know the form you need, talking with Medicare is an option. Also, they have representatives available 24 hours a day, seven days a week.
However, for those on Medicare Advantage plans or Part D Prescription Drug Plans, contact the insurance company for information on which form to submit. Also, your insurance companies contact information is on the back of your insurance card.
